GINGIVAL PLASTIC SURGERY
(2 PEDICLE FLAPS & FRENECTOMY)

Before: X-ray picture of lower front teeth shows nothing abnormal, but photo reveals prominent labial frenum (bottom arrow) pulling on gum (gingiva) between central incisors (top arrow), and perhaps contributing to the receding gums (right arrow) exposing roots (left arrow).
Before
After: 11 months after surgery

Roots are covered by gingiva.
Frenum is gone.

Note: An alternative surgical procedure (free gingival graft) using gingiva from the palate, could have been done to achieve a similar result, instead of the 2 pedicle grafts.

Dr. Perlmutter’s surgical procedure is shown below

Surgical Procedure
Midline soft tissue, including frenum, is cut away.
Split thickness flap, containing labial aspect of 2 papillae has been repositioned closer to the midline.

Surgery is completed. 2 similar flaps have been made and repositioned to the midline.
Green sutures are visible. Periosteum covers the bone at the right and left donor sites.

(No photo was made of the dressing that was placed over this surgical site this day.)

Six days later, dressing and sutures were removed. Site is healing well.
